Claim for Damages
In the event that a city vehicle or piece of equipment causes property damage to a citizen's structure or vehicle such that it may be appropriate to pay repair costs, the citizen is entitled to request a claim for damages. The cause of the damage claimed must be directly attributable to a malfunction of uninsured city vehicles or equipment, neglect or misue of such equipment by city personnel, dangerous disrepair of City infrastructure, or a similar situation. Claims related to disrepair of City infrastructure must not have been caused by factors beyond the control of the city (flooding, tornados, ice storms, etc.), nor by third parties, nor by the failure of the claimant to act in a reasonably prudent manner. If you have a claim related to a City vehicle, this would include claims for rocks from dump truck damages, please contact the Purchasing Office at 479-575-8256.
How to File a Claim:
Complete the "Damage Claim Form" clearly stating the reason for the claim, amount you are claiming, all contact information, and attach appropriate documentation including receipts, estimates, photos, etc. Substantive documentation (written estimates and/or receipts, proof of incident, etc.) MUST be provided to establish all monetary values. Additional sheets may be added. Failure to provide all information and fully substantiate your claim will result in the claim being rejected.
